April 16 is Advance Care Planning Day in Canada: A day to promote conversations about your wishes and values for your future health and personal care.

This year's Advance Care Planning Day is based on the theme Advance Care Planning: Life Happens… Be Ready.

Advance care planning is not just about end of life care, it is about preparing you and your Substitute Decision Maker(s) for a time when you may not be able to make your own health and personal care decisions. At that time, your Substitute Decision Maker(s) would step in to make decisions on your behalf.

Starting these conversations early can alleviate stress on your Substitute Decision Maker(s) and assist you in receiving care that aligns with your expressed wishes.
