Working within the context of a multidisciplinary team, the Social Worker works collaboratively with interprofessional and administrative team members. The Social Workers is knowledgeable about the fields of mental health and addictions, palliative care, and patient navigation. Huron Shores Family Health Team is seeking a highly motivated, self-directed individual to join our multi-disciplinary team at our Blind River site.
Duties
• Providing individual and group counselling and support to patients and families of the Huron Shores Family Health Team.
• Liaise, make referrals and advocate on patient’s behalf within the community as necessary.
• Provide public education, outreach and consultation to professionals or groups.
• Participate in the development and implementation of various programs within the HSFHT.
• Work within a multidisciplinary framework and participate in ongoing training and learning opportunities.
• Represent HSFHT at various Mental Health related community working tables.
• Maintain accurate client records and statistical reports as required.
• Evaluate the effectiveness of counseling and social programs.
• Other duties and projects as assigned.
Qualifications
• Current registration in good standing with the Ontario College of Social Workers and Social Service Workers.
• Bachelor of Social Work.
• Masters of Social Worker considered a strong asset.
• High standards of ethics and confidentiality to handle sensitive information
